If you're a new client, please complete the following forms and submit them before your first therapy session.
- Adult Intake Form (ages 20 +)
- Teen Intake Form (ages 13-19)
- Child Intake Form (ages 8-12)
There are additional signature forms and consent forms that need to be filled out, however you will need to reach out to the office to receive the remaining paperwork.
